A. Renewal of Passport

  1. Original passport with photocopies of the 1st page, last page, and page bearing valid employment pass / visit pass / dependent pass
  2. 3 copies of latest 1.77” x 1.37” photograph (colored with royal blue or dark blue) and applicant attired in decent
  3. B$85.00 or US$50.00 (32 pages) and B$102.00 or US$60.00 (64 pages)

B. Replacement of Lost Passport

  1. Police report (from station with jurisdiction over area where loss was reported)
  2. Statutory Declaration from the Magistrate Court or Affidavit of Loss (B$42.50 or US$20.00) from the Philippine Embassy
  3. Special pass from the Immigration (Enforcement Section)
  4. 3 copies of latest 1.77” x 1.37” photograph (colored with royal blue or dark blue)
  5. 15 days-waiting period before new passport can be issued – this requirement allows the Embassy time to verify and furnish copies of the affidavit of loss and police report to the various authorities in the Philippines
  1. B$153.00 or US$90.00 for replacement of lost passport (cost incorporates penalty, prescribed by regulations of the Department of Foreign Affairs)

C. Replacement of Damaged Passport

  1. Statutory Declaration from the Magistrate Court or Affidavit from the Philippine Embassy (B$42.50 or US$25.00) explaining how the passport was damaged
  2. Damaged passport
  3. 3 copies of latest 1.77” x 1.37” photograph (colored with royal blue or dark blue)
  4. B$85.00 or US$50.00 (32 pages) and B$102.00 or US$60.00 (64 pages)

D. Amendment of Passport (due to marriage only)

  1. Original and photocopy of duly authenticated marriage contract (authenticated by the Authentication Division of the Office of Consular Affairs, DFA)
  2. Passport
  3. B$34.00 or US$20.00

E. New Passport for Minors

  1. Original and 3 photo copies of the Birth Certificate issued by the Registrar of Birth, Deaths and Marriages
  2. Original and 4 photo copies of the duly authenticated marriage contract of parents, if married
  3. Photocopies of the passports of each parent – 1st page, last page and the page bearing valid employment visas.
  4. 3 copies of latest 1.77” x 1.37” photograph (colored with royal blue or dark blue)
  5. 3 copies of Report of Birth ( Forms available at the Embassy)
  6. B$42.50 or US$25.00 for the Report of Birth and B$85.00 or US$50.00 for passport (32 pages), B$102.00 or US$60.00 (64 pages).
  7. Personal appearance of either parent together with the child.  If filed by a third party, parent’s Special Power of Attorney should be presented.

F. Marriage

  1. Certification from the Office of the Civil Registrar General, National Statistics Office, that he or she has no record of marriage, authenticated by Malacañang and the Authentication Division of the Office of Consular Affairs, DFA
  2. Written consent to marry by father/ mother/ surviving parent or guardian, or persons having legal charge, for contracting parties between the ages of 18 and 21 years old.
  3. B$42.50 or US$25.00

G. Special Power of Attorney

  1. Passport of the applicant
  2. B$42.50 or US$25.00

H. Affidavit of Support and Consent

  1. Application form (available at the Embassy)
  2. Passport of the person executing the affidavit
  3. B$42.50 or US$25.00

I. Authentication of Documents

  1. Original documents to be authenticated and one certified true copy by the magistrate court of Brunei Darussalam.
  2. B$42.50 or US$25.00

J. Endorsement for Philippine Driver’s License

  1. Original and photocopy of Driver’s License
  2. Original and photocopy of receipt issued by the Land Transportation Office, Philippines
  3. Original passport and copies of 1st page, last page and page bearing valid employment pass
  4. Application Form (available at the Embassy)
  5. B$42.50 or US$25.00

K. Certification for Travel Tax Exemption

  1. Photocopies of passport of applicant -- 1st page, last page, and page bearing valid employment pass
  2. Photocopy of IC of spouse
  3. B$42.50 or US$25.00

L. Certification for NBI Clearance

  1. NBI form to be fingerprinted at Gadong Police Headquarters
  2. Photocopy of passport of applicant
  3. Application form (available at the Embassy)
  4. Copy of photograph
  5. B$42.50 or US$25.00
